A printable PDF asks every client the same thing

Most massage intake forms online are a PDF you print, hand over on a clipboard, and then retype into your records. It asks everyone the same questions, cannot branch by why they booked, and only gets filled in once the client is already in the waiting room. A form clients open on their phone before they arrive fixes all three.

Ask what brings them in - then tailor the rest

One question routes each client down the path that fits. Someone booking to unwind and someone rehabbing an injury need different questions, so ask once and let everyone get the intake that matches why they booked.

Relaxation track

A short welcome video plus questions on pressure preference and the areas you would like extra time on.

Rehab track

Questions on where it hurts, how long it has been going on, and any recent injuries or surgeries to work around.

Prenatal track

Questions on how far along they are and any pregnancy symptoms, so you can adapt positioning and pressure safely.

Let them show you where it hurts

A text box turns "my lower back has been tight for weeks" into one flat line. Let clients answer with video or audio instead and they point to the spot, describe the ache, and tell you what makes it worse - the detail you would normally dig for in the first five minutes on the table.

Where are you feeling tension or pain right now, and how long has it been going on?

A printable PDF template

  • Printed, filled in on a clipboard, then retyped into your records by hand
  • The same generic questions for every client, whatever they booked
  • Handwriting to decipher, with a contraindication easy to miss
  • No way to ask a follow-up or let them show you the sore spot
  • Only gets filled in once the client is already in the room

A Clipform intake form

  • Opens on any phone from a link or QR code, filled in before they arrive
  • Routes by reason for visit, so relaxation, injury and prenatal clients get the right questions
  • Clients show you where it hurts on video, not one flat line of text
  • Health history and consent captured digitally and legibly, every time
  • Every answer lands in one place, ready to read before the session
