Most exit surveys collect ratings nobody reads. A 1-5 grid tells you engagement dropped; it never tells you about the manager conversation that caused it. This template swaps the grid for a short guided flow: departing employees answer in their own words, by text, voice or video - whichever they are comfortable with.
It asks one question at a time. First the main reason they are leaving, then the moment they started looking, their relationship with their manager, the one thing they would change, and anything they never felt able to say. It ends with an optional follow-up step, so HR can get back to anyone who wants a conversation.
