About this template

A satisfaction score tells you something changed. It never tells you what. This customer feedback form pairs every score with a "why?" that customers answer in their own words - text, voice or video, whichever is easiest for them.

It asks four things: how satisfied they are, the main reason, how likely they are to recommend you, and one thing you could do better. The follow-up step is optional, so customers choose whether you get back to them.
