Create Your First Form

Build and share your first Clipform in under five minutes.

This guide walks you through creating a form using the Clipform dashboard. By the end, you'll have a live form ready to share with respondents.

Each screen in your form is called a node - a question, a welcome screen, or an end screen. You build forms by adding and connecting nodes on a visual canvas. Your work is saved automatically as you go.

1. Create a new form

From your dashboard, click Create Form. Give your form a title - this is what respondents see at the top of the form and what you'll see in your form list.

2. Add your first node

Your form starts with a default end screen. To add a node, open the toolbar on the left side of the canvas and drag a node type onto the canvas.

For your first form, try a Choice node - this lets respondents pick from a set of answers. Once it's on the canvas, click on it to open the settings panel on the right where you can:

  • Write your prompt text in the text field
  • Add answer options
  • Choose between single or multiple selection

3. Add media to your node

What makes Clipform different is video. Click on a node and use the media panel to:

  • Record a video directly from your webcam
  • Upload a video or image file
  • Search stock media using the built-in library

The media plays alongside the node when respondents fill out the form, making it feel like a conversation rather than a static survey.

4. Add more nodes

Build out your form by dragging more nodes from the toolbar onto the canvas. Nodes connect automatically in the order you add them. You can rearrange the flow by dragging connections between nodes, or right-click a node to delete it.

Common form structures:

  • Feedback form: Choice node, Open-ended follow-up, Contact node, End screen
  • Quiz: Multiple Choice nodes with scoring, End screen showing results
  • Lead capture: Button node (welcome), a couple of nodes, Contact node, End screen with CTA

5. Preview your form

Click the Preview button in the toolbar to see exactly what respondents will experience. Walk through the entire form to check that each node flows naturally and media plays correctly.

6. Go live and share

When you're happy with your form:

  1. Click the status toggle in the top-right corner to switch from Draft to Live
  2. Click Share to open the share dialog
  3. Copy the shareable link (e.g. https://clipform.io/abc123) and send it to respondents, or embed it on your website

You can switch a form back to Draft at any time. Draft forms are not accessible to respondents.
