Build a Lead Capture Form
Collect contact details with a video introduction.
A lead capture form introduces you or your product with video, asks a couple of qualifying questions, and collects contact details. This guide walks you through building one.
What you'll build
A short form with:
- A video welcome screen (Button node)
- One or two qualifying questions (Choice nodes)
- A Contact node to collect details
- An end screen with a call to action
1. Create the form
From your dashboard, click Create Form and name it (e.g. "Book a Demo" or "Get in Touch").
2. Add a welcome screen
Drag a Button node onto the canvas. This is the first thing respondents see. Write a short welcome message like "Hey! Let me show you how we can help." and set the button text to something like "Let's go".
Record a video of yourself introducing the form - who you are, what the form is about, and why they should complete it. This personal touch is what sets Clipform apart from a static form.
3. Add qualifying questions
Drag one or two Choice nodes onto the canvas. These help you understand who's filling out the form before they give you their contact details:
- "What best describes your role?" with options like Founder, Marketing, Sales, Other
- "What are you looking for?" with options like Demo, Pricing, Partnership
Keep it short - the goal is to qualify the lead, not interrogate them.
4. Add a Contact node
Drag a Contact node onto the canvas. Choose which fields to include:
- Email (mark as required - this is the minimum you need)
- First name and Last name
- Company and Job title (optional - useful for B2B but adds friction)
- Phone (optional)
Only include fields you'll actually use. Every extra field reduces completion rates.
5. Set up the end screen
Configure the End Screen with:
- A thank-you message ("Thanks! We'll be in touch within 24 hours.")
- A Call to action button linking to your website, calendar, or a resource
6. Preview and go live
Click Preview and walk through the form as a respondent would. Check that the video plays, questions flow naturally, and the Contact node captures the right fields. Toggle to Live and share it or embed it on your website.
Tips
- Keep it under 5 screens - welcome, 1-2 questions, contact, end screen. Shorter forms convert better.
- Record video for the welcome - even a 15-second clip dramatically increases engagement vs text alone
- Make email required, everything else optional - you can always follow up to get more details
- Use the end screen CTA - link to your calendar booking page so leads can self-schedule
